5) Are you on a PC, laptop, phone, or using a tablet?a) If you are on a tablet or mobile device, what version and version of software do you have?

6) What is the exact URL of the website you are having problems accessing?
7) What is the exact error you are receiving?
8) What exactly were the steps you took when receiving the error?

10) Newsletter Issues - If you are having trouble accessing links in the newsletters you received please tell us the following
a) What is name of the newsletter received?
b) What date did you receive the newsletter?
c) What was the title of the newsletter?
d) What email provider did you receive your newsletter at, and list the email address it came to if different that the email you are contacting us from?
e) What block were you clicking to view content?
